Report a claim
We understand that the loss of a loved one can be a very difficult and confusing time. That's why we make it our priority to keep the claims process as streamlined as possible. To report a claim, just complete the steps below.
- Request a claims packet.
Download claims packet or request a claim packet by phone. Out business hours are 8 a.m. to 5 p.m. (ET). (Include W-4P)
- Complete and mail your claim for with a certified death certificate (copies not accepted)

If necessary, a claims representative may contact you to request additional documentation needed to complete you claim. For a complete list, see the Claims Checklist below. If requested, the documents can be amiled to one of the addresses listed above.
Here is a list of documents you will need to submit in order to ues to consider youo claim:
- Complete claim form
- Certified death certificated
- W-4P form
The follow documents may be requested depending on your specific situtation:
- Proof of guardianship for a minority beneficiary
- Executorship papers for the estate
- Funeral home assignment(s)
