Report a claim

We understand that the loss of a loved one can be a very difficult and confusing time. That's why we make it our priority to keep the claims process as streamlined as possible. To report a claim, just complete the steps below.

  1. Request a claims packet.

    Download claims packet or request a claim packet by phone. Out business hours are 8 a.m. to 5 p.m. (ET). (Include W-4P)

  2. Complete and mail your claim for with a certified death certificate (copies not accepted)

  3. Provide additional documentation if needed

    If necessary, a claims representative may contact you to request additional documentation needed to complete you claim. For a complete list, see the Claims Checklist below. If requested, the documents can be amiled to one of the addresses listed above.

    Here is a list of documents you will need to submit in order to ues to consider youo claim:

    1. Complete claim form
    2. Certified death certificated
    3. W-4P form

    The follow documents may be requested depending on your specific situtation:

    1. Proof of guardianship for a minority beneficiary
    2. Executorship papers for the estate
    3. Funeral home assignment(s)

    How do I file a claim?


    To start, call our toll free claims number 1-800-303-6681 to report the death. Our business hours are 8 a.m. to 5 p.m. (Eastern Time), Monday through Friday. A representative will help you complete your claim.
